Patient information & education

Using a mandibular advancement device: 1. The appliance should be individually fitted after dental and medical assessment and, for OSA, used within an appropriate sleep-medicine pathway. 2. Insert it before sleep as instructed; advancement is usually adjusted gradually rather than set to one fixed position immediately. 3. Clean it daily with a toothbrush and suitable mild cleaner; avoid hot water that could distort the material. 4. Store it clean and dry in a ventilated container. 5. Attend follow-up appointments to assess fit, dental side effects, jaw comfort and treatment effectiveness. Temporary salivation, dry mouth, tooth or jaw discomfort and morning bite changes can occur. Long-term use can cause tooth movement or bite changes, which is why dental follow-up matters. Mandibular advancement devices are evidence-based for selected patients with primary snoring and OSA, particularly when prescribed after sleep assessment. CPAP remains first-line or preferred treatment for many patients with moderate to severe OSA, depending on individual circumstances.

Mandibular advancement devices can help selected patients with snoring or obstructive sleep apnoea as part of medical sleep care.

Possible complications include pulpal inflammation, sensitivity, pain on biting, structural fracture and, for deep root cracks, loss of the tooth. Diagnosis may use magnification, transillumination, bite tests, periodontal probing and targeted radiographs. Small defects may be restored conservatively; structurally compromised teeth may need cuspal coverage with an onlay, overlay or crown. Root canal treatment is indicated only when pulpal disease requires it. A deep split or vertical root fracture may make extraction unavoidable.

With long-standing edentulism, the residual ridge may become narrower and flatter, which can make a conventional denture less stable. Implant-supported overdentures or fixed full-arch restorations can improve retention and chewing function in appropriately selected patients. The best option depends on bone volume, anatomy, medical factors, hygiene, dexterity, expectations and cost. Implants do not completely prevent normal bone remodelling, and not every patient needs or is suitable for a fixed solution.
