Jaw Muscle Pain Treatment

Jaw Muscle Pain and Myofascial Disorders in Basel
In brief: Myopathies
Myofascial and other masticatory muscle disorders involve muscular causes such as overload, trigger points or altered function. In Basel, we assess symptoms, muscle tenderness, jaw movement, bite contacts and habits such as clenching or grinding. Treatment is usually conservative: explanation, self-management, an occlusal splint where indicated, physiotherapy and interdisciplinary assessment when needed. The goal is measurable symptom relief through a structured diagnosis rather than vague labels.
Masticatory muscle disorders
in the jaw and facial region are disorders or functional disturbances of the masticatory muscles. In contrast to arthropathies, which primarily affect the temporomandibular joint, the main source of symptoms here is muscular. Typical complaints include pain, tightness, fatigue or restricted jaw function. A differentiated diagnosis is important because treatment depends on the underlying mechanism.

Important types of masticatory muscle disorders
Muscular symptoms and temporomandibular-joint findings can occur together and may need to be assessed as part of the same clinical picture.
May be associated with clenching or grinding, prolonged muscle activity, unilateral chewing or other loading patterns.
Less common in routine dentistry and may occur in the context of systemic disease or infection; medical assessment can be necessary.
Muscle tenderness, trigger points or altered function without obvious structural damage; stress and parafunctional habits can contribute.
May follow injury, surgery or excessive stretching of the masticatory muscles.

Diagnostic methods
Palpation of the masticatory muscles, assessment of jaw function and reproduction of familiar pain where appropriate.
Ultrasound or MRI may be considered when a structural lesion or another diagnosis is suspected; imaging is not routinely required for simple muscular pain.
Measurement of mouth opening and movement, muscle coordination and factors that provoke or relieve symptoms.

Costs and Individual Treatment Planning
Clinical examination and functional assessment are charged according to the applicable Swiss dental tariff and time required.
MRI or ultrasound, if indicated, may involve additional costs and may be provided by an external specialist.
Depends on the diagnosis and may include an occlusal splint, physiotherapy, medication prescribed where appropriate, behavioural strategies or stress management.
Applications in dentistry
Facial pain and muscle tension
Restricted mouth opening
Headache with a possible muscular component
Symptoms associated with clenching or grinding
Muscle symptoms occurring alongside systemic conditions
For the diagnosis and treatment of temporomandibular disorders, we work closely with the TMJ Centre of Dr Kruse Gujer. This interdisciplinary collaboration allows findings and treatment options to be assessed from complementary specialist perspectives.

Diagnostics and Planning
Symptoms involving the masticatory system are often multifactorial. We therefore assess the temporomandibular joints, masticatory muscles, tooth contacts, loading patterns and the course of symptoms together. The aim is a staged approach that is as reversible as possible, with regular reassessment.
Limitations, Risks and Alternatives
A splint or functional treatment will not resolve every type of head, muscle or joint symptom. With atypical symptoms, neurological signs or suspicion of another cause, medical or physiotherapy assessment may be appropriate. Treatment is adjusted according to the clinical course.
Myopathies in Basel: experience and clinical perspective
In our Basel practice, functional symptoms are not assessed solely on the basis of individual tooth contacts. We consider symptoms, muscles, joints and reconstructive factors together and, where possible, begin with reversible measures. Other disciplines are involved when appropriate.More about Dr Thomas Gasser’s training and qualifications.
